Is your child over age 3, but you still have concerns? How to begin the DDSN Services Process!

  1. Call DDSN’s toll free number - 1-800-289-7012 to make a referral.

    • Expect about a 15 min phone conversation to establish SC residency and general information regarding disability.

    • Have the applicant’s full name, birth date, social security number, Medicaid number (if applicable), home address, phone number, and email address available for the phone call.

  2. DDSN will offer you a list of providers to complete Intake. If you feel like we would be a good fit for you and your child, we would love to work with you!

  3. We will contact you to assist you in submitting documentation of your child’s developmental delays

  4. Initial Visit will include an assessment of your concerns and your child’s developmental delays. Our EI will develop goals based off of your concerns and your child’s delays and will also explain additional services your family may qualify for!
